Many physicians in Canada work as independent practitioners (fee-for-service or similar), not as classic employees. That structure collides with normal Express Entry rules—especially Canadian Experience Class (CEC), which usually excludes self-employment. Ontario’s Workforce Priority (OWP) stream and a federal temporary public policy create workable paths if the facts fit.

1. Express Entry: you still need to meet the requirements under CEC, FSWP, or FSTP

Under Express Entry, the “Physicians with Canadian work experience” category is not a fourth program. It only invites people already in the pool who also meet category experience rules (generally ≥1 year Canadian work in the last 3 years in NOC 31100, 31101, or 31102).

2. CEC and the physician public policy

Default rule: Under the Immigration and Refugee Protection Regulations, self-employment is not included in CEC qualifying Canadian work experience [R87.1(3)(b)].

For CEC-related relief, the policy is aimed at foreign national physicians who:

  • Are invited to apply through Express Entry on or after 25 April 2023 and apply under CEC (or certain PNP scenarios under the policy text), and

  • Have acquired in Canada, in the three years before the PR application, one year of full-time (or equivalent part-time) work experience—including work remunerated for publicly funded medical services—as a physician in the listed NOCs (31100 / 31101 / 31102 under NOC 2021).

IRCC profile instruction:
When creating the Express Entry profile, do not check “Self-employed work” under Work experience in Canada if you need that Canadian medical work to count as Canadian experience for this pathway.

The policy can be revoked without notice. All other selection and admissibility rules still apply.

3. “Paid work” vs Self-employment

All three EE programs require paid skilled work—not volunteer or unpaid internships.

Why FSWP can accept self-employment without a special “physician-only” regulation like CEC:
CEC’s regulation expressly excludes self-employment. FSWP’s work-experience rules focus on paid skilled work in an eligible NOC; IRCC’s FSWP guidance lists self-employment among situations that can count for selection factor experience. Fee-for-service physicians who are paid for services are not “unpaid volunteers.” Evidence (billing, contracts, tax documents, practice records) must support genuine paid skilled work.

FSTP is rarely the right vehicle for physicians.

4. Ontario OWP: self-employed physician pathway

Under the Ontario Workforce Priority stream, you may apply without a job offer if you:

  1. Are eligible to receive payment for publicly funded health services (typically an OHIP billing number), and

  2. Hold an eligible CPSO certificate and are in good standing:

    • independent practice, or

    • academic practice, or

    • provisional certificate

Postgraduate education licences are not eligible on this self-employed route (a job-offer OWP path may still be possible).

Employer revenue, staffing, and job-offer rules do not apply. Process is physician-specific (EOI via OINP’s physician process; if invited, application within 17 days). You may request nomination through Express Entry if you also meet a federal EE program.

Bottom line: Under EE, a job offer is optional for many candidates and no longer boosts CRS. Under OWP, a qualifying job offer is central unless you are on the self-employed physician track.

Those cut-offs only help candidates who are already EE-eligible (CEC/FSWP/FSTP) and meet the physician category experience rules. An OINP nomination (+600 CRS) is a different lever and can dominate ranking if issued through EE.

Can a self-employed physician apply to OINP without a job offer?
Yes, under the Ontario Workforce Priority stream, if you have an OHIP billing number (eligible to receive payment for publicly funded health services) and hold an eligible CPSO certificate in good standing (independent practice, academic practice, or provisional). Postgraduate education licences are not eligible on this self-employed route.

Does OINP nomination automatically make me eligible for Express Entry?
No. To use an Express Entry–aligned nomination you must still meet the minimum requirements of CEC, FSWP, or FSTP and keep a valid Express Entry profile. OINP rules alone are not enough.

Can self-employed work count toward the Canadian Experience Class (CEC)?
Usually no. CEC excludes self-employment. Under a temporary public policy, some foreign national physicians can count Canadian work providing publicly funded medical services (e.g. fee-for-service) toward CEC. When building the profile, do not check “Self-employed work” under Work experience in Canada if you need that experience to count. The policy can be revoked; confirm current IRCC instructions.

Can self-employed work count under the Federal Skilled Worker Program (FSWP)?
Yes, if it is paid skilled work that matches your NOC and meets FSWP rules (including the one-year continuous experience requirement and other selection factors). Volunteer or unpaid work does not count. FSWP does not use the same self-employment ban as CEC.

Is a job offer required for Express Entry?
No for entering the pool under CEC, and not generally required for Express Entry. Since 25 March 2025, job offers also give no CRS points. Under OINP Workforce Priority, a qualifying job offer is required for TEER pathways; the self-employed physician pathway is the main exception.

What is the Express Entry “physicians with Canadian work experience” category?
It is a category-based invitation round, not a separate program. You must already be in the Express Entry pool (CEC, FSWP, or FSTP) and meet the category rules—typically at least one year of Canadian work in the last three years in NOC 31100, 31101, or 31102.
